An Opportunity to Develop an Integrated ACG Solution
With the help of our academic partners, Integral Health Solutions is now majoring on incorporating social care needs data into the Johns Hopkins Adjusted Clinical Groups (ACGs) risk stratification tool. This is an innovation for ACGs in the NHS driven by the Integrated Care Agenda. Integral Health Solutions are keen to work with a test-site or number of test-sites where both local authority social care and the GP practices would be willing to collaborate on integrated data into the ACG tool.
Of all the current risk stratification tools available in the UK today, the ACG suite is perhaps the best placed to develop the integration of social care risk factors. Working with our academic partners, we have identified a number of key social care risk factors, which predict both high health care costs (admissions, re-admissions and extended length of stay) and social care costs (probability of nursing home placement, requirement for home care support etc).
Since, combining data from health and local authority social services systems is problematic, we have formulated a more practical solution to creating a workable, patient-specific, integrated ACGs solution. This approach works in harmony with social care and primary care and develops the existing tried and tested ACG data generation and inputting to the model.
